Job Assignment

This position exists to develop and maintain patient Plans of Care for assigned direct care unit(s). The POC Liaison collects assessment data from multiple disciplines as well as personal interviews with patients, analysis data based on established theory and principles of the care and treatment of the mentally ill. The POC Liaison formulates a plan of care document based on patients' assessed needs that are individualized, attainable, measurable and behavioral, exercises independent judgment as to the proper interventions and strategies to include on the plan of care in order to help patients meet their goals. Understands and assures that the documents meet all requirements set by CMHIP policy and accrediting agencies (HCFA/CMS, CDPHE, DMH, JCAHO, etc.). The POC Liaison presents the plan of Care to the interdisciplinary treatment team and patients, clarifies the underlying rationale for the proposed goals, objectives and interventions.

Job Duties:

  • Reviews the patients' course of treatment, interviews patients and clinicians, and reviews the medical record, synthesizing the data in order to develop monthly Plan of Care review documents:

  • The POC Liaison trains others on the use of the Plan of Care documents and how to document to the goals and objectives;

  • Performs analysis of data obtained from assessments and interviews (patients, families and staff) and, based on established theory/principles of treatment of the mentally ill, determines the best course of action and clinical approach to finding solutions to the presenting problems; Collaborates with the treatment team and writes a Plan of Care that has individualized, attainable, measurable and behavioral goals/interventions;

  • Provides general educational materials to patients during POC meetings;

  • Updates the Plan of Care in collaboration with the interdisciplinary treatment team, family input, and other involved clinicians. Collects, summarizes, analyzes and interprets findings from the interdisciplinary assessment database;

  • Interviews patients, families, and other direct care staff members who may not be otherwise involved in the planning meetings. Improves patient care documentation by educating treatment team members how to document effectively to the goals and interventions on the Plan of Care, with notations that specifically address the patients' response and progress;

  • Provides feedback to staff regarding their documentation. Attends team meetings, morning reports, and other meetings as directed;

  • Interviews patients, families and staff in order to gather information needed to formulate and update Plans of Care;

  • Is involved with the verbal and behavioral management of patients in crisis, as necessary;

  • Will attend required trainings and drills, and maintain competency in these areas.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

Minimum Qualifications:

Graduation from an accredited college or university with a bachelor's degree in a field of study related to the work assignment and two years of professional experience in the health professional field or specialized subject area of the work assigned to the position. Certification, registration and/or licensure may be required for some assignments.


Work experience in the health professional field or specialized subject area of the work assigned to the position which provided the same kind, amount and level of knowledge acquired in the required education may substitute for the bachelor's degree.

A master's or doctoral degree from an accredited college or university in a field related to the work assignment may be substituted for the bachelor's degree and at the agency's discretion, one or two years of professional experience, respectively

Work that involves exercising discretion, analytical skill, judgment, and personal accountability, and responsibility for creating, developing, integrating, applying, and sharing an organized body of knowledge that characteristically is uniquely acquired through an intense education or training regimen at a recognized college or university; equivalent to the curriculum requirements for a bachelor's or higher degree with major study in a pertinent to the specialized field; and continuously studied to explore, extend, and use of additional discoveries, interpretations, and application and to improve data, materials, equipment, applications and methods.

Conditions of Employment:

  • A pre-employment criminal background check will be conducted as part of the selection process as this position is deemed to have direct contact with vulnerable persons.
